Holidays)
between 0900 to 1700 Hours.
Our

When you come for your appointment
Remember to get the following :
 Medicare
or the veteran affairs card.
Referral
letter from your GP, family physician or other doctor.
Have
your insurance information with you.
Bring
copies of records and x-rays, MRIs, CT scans, etc. from clinics
outside.
You may be able to avoid repeating a study if your information is
here. You may also arrange to
have the records sent here prior to your appointment.
To cancel an appointment:
Please call on 02 9411 3177 during office hours and let us
know at least 24 hours in advance so we can
give that time to another patient.
We understand that your time is valuable, and we make every effort
to run on time; however,
emergencies occur that cause schedule delays. We apologize in
advance if we keep you waiting.
